Everything you need to send HDT Dental Lab a case: the Rx form, what to include, and delivery or office pickup.
How it works
Send your first case in three steps

Fill in the Rx form
Download and print our prescription form. Mark the tooth numbers, restoration type, material and shade, then sign it.
- 2
Pack your case
Put the impressions or models, bite registration and the signed Rx form in the case box.
- 3
Contact us
Call or email us to arrange delivery, or we can pick the case up from your office. We make the restoration and return it to you, typically in about two weeks.

Print the form, fill in the tooth numbers, restoration type, material and shade, sign it and include it with every case.

What to include
- Completed and signed Rx form
- Impressions or poured models
- Bite registration
- Opposing model or impression
- Shade, plus photos if you have them
- Implant cases: implant system, platform size and components
Office pickup
We collect cases from dental offices in the New York City area. Call or email us to schedule a pickup.
Turnaround
Typical turnaround is about two weeks from when we receive the case. Complex implant or full-arch cases may take longer; we'll confirm the date with you.

Patient information
Patient details belong on the Rx form that travels with the case. Please don't send them through the website contact form.
