Working with HDT Dental Lab is simple. Here's how a case gets from your office to ours.

Fill in the Rx form
Download and print our prescription form. Mark the tooth numbers, restoration type, material and shade, then sign it.

Pack your case
Put the impressions or models, bite registration and the signed Rx form in the case box.
- 3
Contact us
Call or email us to arrange delivery, or we can pick the case up from your office. We make the restoration and return it to you, typically in about two weeks.
